New Patient Medical Information and Forms
In order to get the most out of your first appointment with Dr. Tayler, please bring or send the following information and forms at least 48 hours in advance of your appointment, if possible:
Test Results and Other Medical Records:
If you have been under a doctor's care or otherwise obtained medical tests within the last several years, please bring all test results, and related interpretations and diagnoses, with you . If the results are in the form of films or other media, they can be emailed to Dr. Tayler at drtayler@whhas.com or mailed to PO Box 68255, Tucson, AZ 85737. This is not essential, but it will help Dr. Tayler treat you most effectively and avoid delays in treatment that can be caused while waiting for critical test results to be obtained.
Wildhorse New Patient Forms:
Fill out each form listed below and send it to Dr. Tayler: click each form to open it, enter your information, click the E-mail Form button in the upper right corner of the form, and follow the instructions to send the .xml file to Dr. Tayler. In addition (or instead, if you are unable to send the .xml file), please print a copy of each form and Email or Mail it to Dr. Tayler using the instruction below as a backup before you close the online form or your changes will be lost.

Print your completed forms, or print out blank copies of the forms and fill them out by hand, and send them to Dr. Tayler using one of the following secure options:

- MAIL: Mail each completed form to Dr. Tayler at PO Box 68255, Tucson, AZ 85737
All methods of sending information described above are secure and only Dr. Tayler will have access to your medical records, which are kept secure and confidential and never released without your written consent.
