New Patient Medical Information and Forms

In order to get the most out of your first appointment with Dr. Tayler, please bring or send the following information and forms at least 48 hours in advance of your appointment, if possible:

Test Results and Other Medical Records:

If you have been under a doctor's care or otherwise obtained medical tests within the last several years, please bring all test results, and related interpretations and diagnoses, with you . If the results are in the form of films or other media, they can be emailed to Dr. Tayler at drtayler@whhas.com or mailed to PO Box 68255, Tucson, AZ 85737. This is not essential, but it will help Dr. Tayler treat you most effectively and avoid delays in treatment that can be caused while waiting for critical test results to be obtained.

Note for Medical Marijuana Evaluations: The State of Arizona requires Dr. Tayler to evaluate your medical records as part of your evaluation. So, you MUST have BOTH your last 12 months of medical records related to your condition (if any exist) AND related medical records verifying your diagnosis if that was prior to the last 12 months. If you do not have medical records verifying your diagnosis, then you may be required to have additional diagnostic blood work and/or imaging and you will have to schedule a follow-up appointment with Dr. Tayler ($147) in addition to your initial appointment to complete the evaluation process. If you do not have insurance, the costs for related blood tests and imaging studies are normally around $150. If you have insurance, it will be more cost effective for you to visit your regular doctor for a proper diagnostic workup of your condition before you make an evaluation appointment with Dr. Tayler.

Patients who are scheduled for the Initial Homeopathy/Naturopathy 3-Appointment Series, please also complete and send or bring the following additional intake information forms to Dr. Tayler: (MEDICAL MARIJUANA Patients do NOT need the forms below)
